Authorization

We hereby authorize and instruct ACTS, or its duly authorized agents, to:
  • Obtain and review your credit report, and to
  • Request verifications of your income, rental history, and any other information deemed necessary for meeting your housing goals.
Your credit report will be obtained from a credit reporting agency chosen by Acts Housing. You understand and agree that Acts Housing intends to use the credit report for the purpose of evaluating your financial readiness to purchase and/or to engage in post-purchase Coaching activities. You further authorize Acts, or its duly authorized agents, to share your credit report and any information that you provide (including any computations and assessments produced) with the entities listed below to determine your viable financial options.
  • Lenders   
  • Banks    
  • Mortgage Servicers   
  • Debt Collectors   
  • Landlords

  • Public Housing Authorities
  • Social Service Agencies
  • Counseling Agencies
  • Employers


Entities, such as mortgage lenders, may contact your coach to evaluate the options for which you may be eligible. In connection with such an evaluation, you authorize the credit reporting and/or financial institutions to release information and cooperate with your Acts Housing Coach. No information will be discussed about you with entities not directly involved in your efforts to reach your housing goals.

You hereby authorize the release of your information to program monitoring organizations of Acts Housing, including but not limited to, Federal, State and nonprofit partners for program review, monitoring, auditing, research, and/or oversight purposes. You also agree to keep Acts Housing informed of any changes in address, telephone number, job status, marital status and other conditions which may affect your eligibility for a program or Coaching service that you are seeking.

This form will also serve as authorization. You may revoke consent to these disclosures by notifying Acts Housing in writing.

Services

ACTS offers objective and professional guidance to assist you in meeting your housing goals

Personal Finance Services:

  • Assessment of your financial readiness to purchase
  • Review of your credit without an impact to your scores
  • Development of your personal plan in order to reach your goal
  • Resources to improve credit scores and increase savings

Homeownership Services:

  • Guidance in determining how much of a home you can afford
  • Assessment of qualification for financial incentives towards purchase, down payment and closing costs
  • Identifying mortgage lenders, services and/or products that meet your needs and objectives
  • Preparation and submission of your documentation for pre-approval and mortgage financing
  • Referrals to an experienced network of homeownership professionals; lenders, real estate agents, home inspectors, insurance agents and more
  • One on One Housing counseling throughout the home purchase process

Real Estate Brokerage:

  • Assistance in developing a comprehensive list of homes meeting client housing needs and affordability
  • Preparation of sales contract and guidance with negotiations
  • Coordinates all aspects of the purchase from sales contract to closing
  • Coordinates and attends home showings
  • Develops buying strategies

Rehab Counseling Services:

  • Guidance throughout the repair process of the home you purchase
  • Assess to ACTS rehab loan/grants in order to make home repairs and improvements (if eligible)
  • Technical assistance with project planning and scope of work preparation
  • Assistance with finding experienced home improvement contractors
  • Assistance with understanding City home repair rules and regulations (required permits, building codes, etc…)
  • Assistance with rehab project financial management
  • Coaching and educational resources to help homeowners who want to do it themselves “DIY or sweat equity”

Homeownership Preservation Services:

  • Support to help you sustain successful homeownership
  • Guidance to determine best options in order to avoid foreclosure

I Understand that Acts provides information and education on numerous loan products and housing services, and I further understand that the housing counseling and/or education I receive from Acts in no way obligates me to choose any of these particular loan products or housing programs. I understand that I may be referred to other housing services to assist with concerns that have been identified. I also understand that I am not obligated to use any of the services offered to me.

Disclosure and Marital Property Statement



Acts Housing is a nonprofit Corporation which provides Home buying coaching services, Real Estate Brokerage services, and Rehabilitation services. In addition, Acts provides homes for sale to qualified clients and provides small rehabilitation loans/grants to assist in the repair of these homes. Finally, Acts, in collaboration with other institutions and banks, provides Down Payment and Closing Cost grants to qualified buyers. Acts discloses any fees for services provided, the homes sold, loans and grants are provided under fair and open procedures.

Acts staff does NOT approve or deny mortgage loans from banks nor does it contract for any rehabilitation work. Employees of Acts are not employees of, independent contractors for, nor do they have any separate monetary relationship with any bank, construction firm, appraiser, title company, insurance company, or home inspection company.

If the client chooses to select Acts to provide real estate services, one of our agents will provide you with agency disclosure forms as required by law. Acts discloses that each Realtor, who provides real estate services to you, may receive a commission on the sale of a property to you. In addition, Acts receives a fee from participating banks for Coaching services after most loan closings.

ACTS Housing discloses that each client is free to select any agency to perform any services needed to purchase and/or rehabilitate a home. Upon request, ACTS will provide a client with names of agencies which have provided services to other clients. ACTS does not warrant that these agencies will provide satisfactory services. Each client must ensure for themselves that the agency he/she selects will be competent.

ACTS Housing discloses that Deatra Kemp, Benjamin Sheehan, Kelly Andrew, Dorothy York and/or Michael Gosman, together with your Coach, may participate in the decision to provide you with a grant. Please let your coach know if you have family or business ties to any of these people? Acts discloses that most homes in our neighborhoods contain lead paint. I have received a copy of the pamphlet “Protect Your Family from Lead in Your Home” from ACTS Housing.

Marital Property Statement

Where it applies, no provision of a marital property agreement (including a Statutory Individual Property Agreement pursuant to Sec. 766.587, Wis. Stats.), unilateral statement classifying income from separate property under Sec. 766.59, or court decree under Sec. 766.70 adversely affects the creditor unless the creditor is furnished with a copy of the document prior to the credit transaction or has actual knowledge of its adverse provisions at the time the obligation is incurred.

Homeownership Coaching and Home Inspections

Acts Housing offers pre purchase homebuyer Coaching which covers the following topics relevant to each family:
  • Decision to purchase a home
  • Selection and purchase of the home
  • Issues which may arise during home ownership, including refinancing and mortgage default
  • Final disposition of the home, including sale
Coaches cover these topics during one-on-one sessions with families. These topics are also covered as part of the web-based coursework offered. The families may also be provided links to relevant HUD websites for additional information.

All families participating in Coaching review and are provided with home inspection materials during their sessions. All clients acknowledge receipt of these documents.
